This pad of 100 forms in NCR duplicate format allows for a quick and accurate method in recording any ‘installation’ services you provide.

We encourage you to use this format with every installation as a record for the client. The disclaimer serves as a reminder to the client that they need to take a responsible approach to the day to day use of their products. How to use:- Enter the vehicle details- Customer details and- Date of service- Place the service description code / codes (that best describes what service you have provided) in the relative seat position on the vehicle plan at lower left.- Strike out seat positions that do not apply for that particular vehicle. Parts or components can be entered on the right hand side along with service charges and Total (GST included) at the bottom.- Have the Customer sign at the bottom:  NB: Ensure that you stamp each form with your company details including ABN: number.- The ‘Additional information’ section may be used for many functions.
